Intake Forms

First time clients will receive an automated email requesting them to fill out our digital intake form. If you are able, please fill this out prior to arrival to save time and to allow your therapist to look over your information beforehand. If you are unable or prefer to fill out a paper form, a paper form will be provided upon your arrival (NOTE: This may cut into your session time) or you can print the form, fill it out, and bring it in when you come for your first session.

Cancellations

Clients are required to inform their therapist of a need to cancel or reschedule an appointment no less than 12 hours before their appointment’s scheduled time. This gives me time to reschedule that spot and helps utilize my time effectively while also giving other clients a chance to get in sooner. If you do not notify me at least 12 hours prior to your scheduled appointment time, I will charge you for 50% of the session cost so I can be paid for my time lost.

I understand that sometimes there are emergencies involving health, children, work, etc and do make exceptions for those. I hope to receive the same courtesy from my clients when I, myself have such emergencies.

Sharing Bulk Rate Packages

We offer bulk rate packages (see our Specials page) and allow them to be shared with a spouse or significant other. When you purchase a bulk rate package, please let us know if you want to share and with whom so we can add a note on both accounts.

Minors

We welcome clients under the age of 18 with written permission from their parent/guardian. If they are under 16, their parent/guardian must be present in the room with them. Due to required parent/guardian signature, we will need you to fill out a paper form. You can fill this out when you arrive (NOTE: This may cut into your session time) or you can print our form, fill it out, and bring it in when you come for your first session.

Animals

I love furry friends but in accordance with the Missouri Board of Therapeutic Massage, I can not allow pets in the treatment rooms. Service animals are ALWAYS welcome!

Inappropriate Behavior

I expect clients to behave appropriately during their time with me. If a client speaks suggestively, requests illicit or sexually suggestive services, touches themselves sexually, tries to grab me, purposely exposes themselves, etc, I have the right to end the session early and ask the client to leave. The client will be invoiced for the full price of the session and banned from returning to our office. Depending on the severity of the offense, we may also involve local law enforcement.

Payment

We accept cash, checks written out to “Nerdy Massage”, Venmo payments to @NerdyMassage, and credit card payments.

• ALL credit card payments will include a $3.85 convenience fee. Clients can avoid this fee by paying for services and tipping with cash, check, or Venmo (exception: first-time clients are required to prepay with card but after they’re established, they’ll have these other options)

• All bulk rate package specials must be purchased with cash, check, or Venmo payment only

Location

My location is a home office in Gladstone. For safety reasons, I will not post the exact address publicly. Clients should check their confirmation and reminder emails for a link that contains the exact address, parking details, and images of the exterior so they can confirm they’re at the correct house.
